Note to readers: This is the second post in our Powered by Partnerships series exploring a targeted four-channel approach to digital health.

Let’s face it—digital health is no longer just an innovation buzzword. It’s here, and it’s changing the game for both patients and healthcare teams. Think about it: remote monitoring, telehealth, personalized care apps—these are tools that help patients stay on top of their health while giving clinicians the power to make quicker, smarter decisions. The result? Happier, healthier patients and more efficient, less-stressed providers. Digital health programs are making it easier to tackle complex healthcare challenges while keeping everything running smoothly behind the scenes. If you’re not on board with these innovations yet, it’s time to see what all the excitement is about!

The Importance of Digital Programs in Healthcare

Digital health programs improve patient interaction and adherence to treatment plans by offering easy access to information, remote monitoring, and tailored care. These tools bridge healthcare providers and patients, empowering patients to manage their health actively.

Engaged patients tend to experience better outcomes, and studies consistently show that higher engagement leads to improved medication adherence, chronic disease control, and patient satisfaction.

Xealth integration facilitates the scalability of these programs by enabling seamless deployment of digital health tools within existing healthcare workflows. Through Xealth’s platform, healthcare providers can prescribe and monitor these programs at scale, ensuring patients receive consistent, personalized care across diverse settings and conditions, driving better engagement and outcomes.

MedBridge: Empowering Patients Through At-Home Exercise Programs

MedBridge provides a patient education platform focused on rehabilitation. Offering resources like instructional videos, articles, and interactive modules, MedBridge helps patients better understand their conditions and adhere to treatments.

Allina Health turned to Xealth to automate the enrollment process for the PT/OT/Speech options to increase patient utilization and improve the provider workflow by enabling the creation of an HEP direct from the Digital Care section of the EHR.

The results:

  • Clinicians saved two to three minutes per monitor view.
  • Across 20,000 clinician monitor views, these savings amounted to $120,000 to $250,000.

Babyscripts: Revolutionizing Pregnancy and Postpartum Care

Babyscripts is a digital health platform focused on maternal care. It offers remote monitoring to ensure the health and safety of mothers during pregnancy.

Baystate Health partnered with Babyscripts during the COVID-19 pandemic to launch a remote blood pressure monitoring program.

The results:

  • Enrollment: > 1,000 patients
  • 84% engagement rate
  • Significant reduction in postpartum readmissions

The program proved highly effective. As a result, Baystate is expanding the initiative across the health system to provide enhanced monitoring and care for pregnant patients before and after childbirth.

SilverCloud: Delivering Accessible Digital Mental Health Solutions

SilverCloud is a digital platform providing mental health support through therapies like cognitive behavioral therapy (CBT).

Banner Health added SilverCloud to its Digital Therapeutics formulary via Xealth to improve access to behavioral health resources, making it accessible to primary care practices.

Since its implementation:

  • Users have engaged with the platform for an average of two hours — surpassing the initial goal of 60 minutes.
  • Additionally, clinical outcomes have exceeded the goal of 50% improvement, with a 54% increase in PHQ9 scores and 58% in GAD7 scores.

These results encourage Banner to expand SilverCloud’s availability to more providers.

PatientIQ: Driving Engagement Through Data Analytics

PatientIQ uses data analytics to personalize patient care and enhance engagement. The platform tailors interventions to the individual by analyzing patient data and improving adherence and outcomes.

In modern healthcare, data-driven tools like PatientIQ enable providers to deliver personalized, efficient care. This data-centric approach ensures patients receive interventions most relevant to their needs, fostering long-term engagement.

Twistle: Seamless Communication for Improved Patient Outcomes

Twistle, a patient engagement platform, helps healthcare providers automate communication and improve care coordination.

Notable results include

  • 16.5% reduction in complications for spinal surgery
  • 5% reduction in the average length of hospital stay

At the University of Rochester Medical Center (URMC), primary care physicians faced challenges in discussing colorectal cancer (CRC) screening during limited appointment times, leading to inconsistent patient experiences.

URMC turned to Xealth and Twistle to segment patient populations and automate outreach. Xealth and Twistle send personalized information on Cologuard, colonoscopies, and FIT tests to appropriate patients through a batch file system.

This scalable solution improves quality measures, saves clinicians time, and ensures all eligible patients receive consistent preventive care information.

Strategies for Scaling Digital Programs

To fully realize digital programs’ potential, personalized interventions must meet diverse patient needs. Customizing these tools based on individual preferences and health conditions can lead to better engagement and adherence.

Integration with existing healthcare systems is also essential for seamless care coordination. By aligning digital tools with traditional care models, healthcare providers can create a more cohesive, efficient system that improves the patient experience.
