Project: Dana-Farber Cancer Institute (DFCI) built a robust Enterprise Content Management System (ECMS) where its internal team curates patient education materials. Using Xealth, DFCI now allows clinicians to seamlessly order these educational resources from within Epic and share them directly with patients through Xealth’s distribution and monitoring tools. This integrated approach not only saves time but also ensures that patients receive relevant information aligned with their care journey.
Outcomes: In just two months, over 500 pieces of content have been delivered to 275 unique patients, with an impressive engagement rate exceeding 40%.
Ureteroscopy Patient Education and Engagement
Objective: Improve patient satisfaction and reduce unplanned encounters around ureteroscopy.
Project: For patients diagnosed with kidney stone disease (KSD) scheduled for or having recently undergone ureteroscopy, an Academic Medical Center implemented a Xealth-supported program to deliver educational content and reminders through MyChart. These messages are timed to help patients better understand their care, manage post-procedure expectations, and potentially avoid unnecessary clinic or ER visits.
Expected Impact: By providing timely education, the Academic Medical Center aims to improve patient and provider satisfaction while reducing unplanned pre- and post-procedure visits.
Advancing Early Detection of Dementia with Linus Health
Objective: Enable earlier detection of dementia and improve access to neuro resources.
Project: At a midwest health system, Xealth supports an integration with Linus Health’s digital cognitive assessment, eliminating manual data entry by automating orders and writing assessment results directly back into the EHR. Linus Health’s platform leverages AI and neuroscience expertise to screen for cognitive impairment, providing rapid and actionable insights into brain health.
Outcomes: Since launching three months ago, over 250 digital cognitive assessments have been completed, marking a 30% increase in usage compared to the prior period, facilitating earlier identification and intervention for cognitive concerns.
Mass General Brigham (MGB): Scaling Breast Surgery Outcomes Monitoring with imPROVE
Objective: Drive adoption of PROMs tracking in breast surgery to meet a 40% patient enrollment target.
Project: MGB’s Division of Breast Surgery has launched the imPROVE app to streamline the collection of Patient Reported Outcome Measures (PROMs) from patients undergoing breast surgery. Thanks to a research grant, Xealth was engaged to integrate the imPROVE app into Epic, facilitating a smooth onboarding experience. The system’s initiative encourages surgeons to enroll at least 40% of eligible patients as part of their annual performance goals.
Expected Impact: Through integration with Epic, MGB anticipates improved adherence to PROMs collection, bolstering both surgical and patient experience metrics.
Montage Health: Increasing Utilization of 24/7 Virtual Visits for Employees
Objective: Increase awareness and use of 24/7 virtual visit options to reduce ED strain and primary care workload.
Project: Montage Health offers a 24/7 virtual visit benefit under its Aspire employee health insurance plan. To improve utilization during high-demand times, such as flu season, Montage launched an awareness campaign using Xealth. Targeted messaging informs employees of the virtual visit options available, aiming to reduce emergency department usage and lessen primary care demands.
Outcomes: By strategically educating plan members about their virtual visit options, Montage Health anticipates higher engagement with its 24/7 virtual service during peak utilization periods.
Scaling Digestive Health Management with FoodMarble
Objective: Streamline ordering of breath tests for digestive health, reducing manual effort and supporting patient management.
Project: A west coast Academic Medical Center currently prescribes the FoodMarble breath test to patients needing digestive health support. To make the workflow more scalable, the health system partnered with Xealth and integrated FoodMarble’s API 2.0 to automate the ordering and tracking process within Epic. Clinicians can now order breath tests directly, triggering notifications that facilitate faster patient care coordination.
