Once digital tools are ordered, Xealth will integrate them to your patient portal (whether stock from the EHR vendor or a customized one), allowing patients to engage with the asset. As patients use these programs, Xealth tracks their engagement, provide the engagement or monitoring details to clinical users so that they can keep up with the patients’ progress and drive interventions accordingly, and even embed third-party dashboards inside of Xealth for viewing from within the EHR
